Community Autism Support Service Referral Form

Thank you for your interest in referring to the Community Autism Support Service.

Please note this referral form is for professionals only, as children and young people must be referred by an appropriate professional as listed below:

• Coventry and Warwickshire’s Partnership Trust (CWPT) Neurodevelopment Team including Occupational Therapists, Speech & Language Therapists and Neuro Clinicians.
• SENCOs
• Early Help and Targeted Support Teams
• Coventry and Warwickshire Operations Team – social workers and social care practitioners
• Mental Health in Schools Teams
• Social prescribers

If you are not one of the above professionals, we are unable to process your referral.

Please be aware that on page 3, you will need to provide a professional supporting statement from an approved professional in the reason for referral box.

This support needs form is for those who need non-clinical targeted small-group support that can help with understanding autism and how it can influence your life.
